Quarter 1 (Q1) 2026 Data — Due April 14, 2026

Quarter 1 of 2026 ended on March 31, 2026. To stay on track with upcoming deadlines, please plan to send your Quarter 1 data to us no later than April 14.

2026 Contracts & Information Sheets

Before any data can be submitted, we will need a signed contract on file. Contracts are being sent out this week and next (4/2/26- 4/10/26), so please keep an eye out for yours!

For 2026, contracts are being sent through a new program, and the signature page will come directly from an AffordableQR email address. Once your contract has been sent, we will follow up with you individually to ensure it was delivered successfully. Thank you for your patience as we work to update our processes.

Additionally, 2026 MVP registration is now open. If you plan to participate in the program for the upcoming year, please ensure that the correct MVP is listed on your information sheet at the bottom of the AffordableQR contract before signing. Once we have received verification through your contract signature we will begin with registration!
