Final 2025 CMS Submissions — Due March 31, 2026

Final submissions for the 2025 reporting year must be sent to CMS no later than March 31, 2026. To keep everything on track, please sign your final approval by March 10, 2026.
This allows AffordableQR enough time to:

• Complete all required quality checks
• Make the final CMS submissions early
• Resolve any issues that may arise before the hard CMS deadline

Your timely approval is critical to ensuring a smooth and successful submission process.

2026 Contracts & Information Sheets — Coming Soon

We appreciate your patience as we finalize and close out the 2025 books.
Once that process is complete, 2026 contracts and updated information sheets will be sent out.

Quarter 1 (Q1) 2026 Data — Due April 14, 2026

Quarter 1 of 2026 ends on March 31, 2026.
